Motor carriers must keep a qualification file for each CMV driver they employ.

Under 49 CFR 391.51 a driver qualification file includes the employment application, motor vehicle records (at hire and annually), safety performance history from previous employers, the road test certificate or equivalent, the medical examiner's certificate (for CDL holders, the CDLIS motor vehicle record showing medical certification status) and the note of the annual review of the driving record.

The file must be kept for as long as the driver is employed and for three years afterwards; some items may be removed three years after their date.
